For the past few years, readers in North America have preferred stories set in Regency England, between 1812 and 1830. I think all the recent adaptations of Jane Austen's books, especially Pride and Prejudice starring Colin Firth, are at least partly responsible for that.

However, Harlequin Historical is publishing books set in many different places as well as times, and they've always continued to publish medievals (yeah!). They recently had a book out in Britain and North America by Michelle Styles called The Gladiator's Honor, set in ancient Rome. It got excellent reviews here.
